IV Use of Name:
The City of Stillwater agrees to allow The Sand Creek•Grou

V Eligibility'
All employees of The City of Stillwater and members.of their household who are experiencing
personal, 'emotional, family or social problems which have •irect, indirect or potential. effects on
work performance or personal well being are eligible for th , services of the Employee Assistance
Program.
VI Employee Participation
A working policy of the EAP is that Employee participation 'n the EAP will affect neither future
employment or advancement nor protect employeefrom dis•iplinary a ction for continued
substandard performance.

X Modification or Termination of Contract:
This contract may be modified in writing at any time by the utual . consent of the parties:
Either The :City of Stillwater or The Sand Creek Group, Ltd may cancel this agreement at any time
upon 90 day notice, in writing, to the other party.
