Shelton `(Name of incumbent) 216 North Fourth Street (Public Entity) Director of Public Works (Official Position) (Addre.e.;) Stillwater Minnesota (City) 439-6121 (7'rlr phunr ,\'u.) 55082 (State) (l_ip Cock) is hereby authorized to execute for and in behalf of City of Stillwater , a public entity established under the laws of the State of Minnesota this application and to file it in the appropriate State office for the purpose of obtaining certain Federal financial assistance under the Disaster Relief Act (Public Law 288, 93rd Congress) or otherwise available from the President's Disaster Relief Fund. THAT City of Stillwater , a public entity established under the laws of the State nf_.
